Promotora Program
At La Red Health Center, we offer community outreach through bilingual Community Health Workers (CHWs) and promotoras, who are certified medical interpreters. This team helps patients get care at La Red and other medical offices. For more than 23 years, our Promotora Program has helped make sure language doesn’t stop anyone from getting quality health care. The promotoras guide patients through appointments and offer support with health education, referrals, and resources. Our bilingual promotoras work in Sussex County to help people access care at La Red and at medical offices without bilingual staff.

Rural Re-entry: A Fresh Start — Health and Support for Residents Returning from Judicial Settings
At La Red Health Center, we believe that everyone deserves a second chance — and access to quality health care is a vital part of rebuilding a healthy, stable life. Our Rural Re-entry Program is designed to support individuals transitioning back into their communities after incarceration, ensuring they receive the medical care, behavioral health support, and resources needed for long-term success.

Re-entering society comes with challenges, but no one should face them alone. Our program connects returning citizens with:
- Comprehensive primary care — Preventive health screenings, chronic disease management, HIV/STI testing, and routine check-ups.
- Dental care — La Red offers affordable dental care at our offices in Milford and Georgetown.
- Behavioral health services — Behavioral health counseling and substance use disorder treatment, including medications for opioid use disorder (MOUD).
- Care coordination and case management — Assistance with Medicaid enrollment and social services.
- Peer support and community partnerships — A network of advocates and mentors who understand the journey.

Homeless Connection to Care
At La Red Health Center, we believe that everyone deserves compassionate care, including those experiencing homelessness. Our dedicated Case Managers go beyond just providing services — they build trust, offer support, and connect individuals with the care they need to improve their well-being.

Here’s how we make a difference:
- We meet people where they are — whether in schools, businesses, churches, or community gathering places, we offer them help and support.
- We take the time to listen and understand each person’s unique needs, and create a plan that puts them on a path to stability and health.
- We assist with gathering medical and job history and scheduling essential health care appointments.
- We provide ongoing support through visits, phone calls, and home check-ins, ensuring no one feels forgotten.
- We help with transportation to the health center, so access to care is never a barrier.
- We advocate for our clients, making sure vital resources reach those who need them most.
- We stay informed and collaborate with community partners to find the best ways to support those facing homelessness.
- Most importantly, we welcome everyone with dignity, respect, and open arms — because at La Red, you are never alone.
Our mental health team also collaborates with doctors and nurses to create a full care plan for each patient experiencing homelessness.
If you or someone you know needs support, we’re here to help. You can access our behavioral and mental health services at any of our care centers in Georgetown, Milford, or Seaford.
If you are a new patient or don’t have access to your patient portal, you may also schedule an appointment by calling or visiting the care centers closest to you.
Delaware WIC On-Site
The Women, Infants, and Children (WIC) program is a federal program that helps families get healthy food, learn about nutrition, and find health care and community services. La Red Health Center offers referrals for patients to get into the program. La Red Health Center’s Georgetown campus is one of the few locations where individuals can meet on-site with a Delaware WIC specialist.
To join, participants must meet income guidelines and be pregnant women, new mothers, infants, or children under age five.
Help Fight Food Insecurity — Support La Red’s Community Refrigerator Program
At La Red Health Center, we believe good nutrition is the foundation of good health. But for many of our patients, access to fresh, healthy food isn’t guaranteed. That’s why we’ve created Community Refrigerators & Freezers at our Milford, Georgetown, and Seaford offices — ensuring that nutritious food is available to those who need it most.
Thanks to the generous support of the Delaware Department of Agriculture and the Delaware Division of Public Health, we were able to launch this vital program.
Through donors and grants, we’ve partnered with local farms to provide fresh produce through community-supported agriculture (CSA) shares. We offer produce bags filled with fresh fruits and vegetables, when available, to patients at high risk for obesity, hypertension, diabetes, and other chronic illnesses.

Mobile Health Unit
La Red Health Center’s Mobile Health Unit brings accessible, affordable, and convenient health care directly to Sussex County residents — wherever they are. Whether you are uninsured, underinsured, or simply looking for a new medical home, our Mobile Health Unit provides a welcoming entry point to high-quality care. We offer a range of essential services, including:
